BAPTISMAL REQUIREMENTS:

  1. Needs certification from their respective Area/Street Coordinators (secure form from the parish office).
  2. Submit the Following:
    • Certified copy of Birth Certificate (original and photocopy) with Registry number.
      • For those born in Metro Manila: secure copy from the local Civil Registry
      • For those born outside Metro Manila: secure certification from NSO
    • Marriage Contract of parents (original and photocopy)
    • Promissory note to marry, if not married
  3. FOR CASES WITHOUT BIRTH CERTIFICATE, submit
    • Certificate of No Record from NSO   or
    • Certified True Copy of Hospital Records
    • Notarized affidavit explaining why there are no records.
  4. Attend the seminar for both parents and one pair of godparents (no proxy) the Saturday before Baptism (4:00-6:00 pm).
  5. Godparents must be Baptized Catholics who have received the Eucharist, Confirmed and not less than 16 years of age.

Note:

  1. No special schedule for Baptism.
  2. For Mary Immaculate Parishioners/Residents ONLY per Vicariate Circular No. 2003-09 dated 29 July 2003:

“Effective 03 August 2003, all Baptisms will be administered only within their respective parishes. No baptism will be accepted outside the respective parishes within the vicariate as agreed upon by all the parish priests of the Holy Family Vicariate which include the following: Holy Family, San Isidro Labrador, Our Lady of the Pillar, Five Wounds of Jesus, Mary Immaculate, Mary, Mother of the Church and Sto. Cristo.”

 CONVERSION REQUIREMENTS:

  1. Original copy of Baptismal Certificate (from other faiths)
  2. Certified copy of Birth Certificate (original and photocopy)
  3. Marriage Contract of parents (original and photocopy)
  4. Must attend a Catechism seminar

BAPTISM SCHEDULE:

BAPTISMAL CERTIFICATE:

Note: Certificates will not be released if requirements are incomplete.

  1. Issued on request at least 2 weeks after baptism.
  2. If claimed by a representative, present letter of authorization from the parents with their corresponding IDs and the authorized person’s ID.
  3. For partial change/reconstruction of records, submit Special Power of Attorney for the representative if parents of the baptized person is outside Metro Manila or abroad
